Part 1 (LO1)
LO1 Examine the influence of theoretical models and methods of assessment and planning in practice in the workplace.
Scenario:
You are working as a senior carer in a healthcare setting. The staff team have recently been reviewing their care plans and the approaches they use in making assessments of needs and developing appropriate care plans.
Activity 1:
In order to develop this process, the manager has asked you to produce a report to share at a staff meeting that evaluates the influence of different theoretical perspectives on models of planning and assessment used in this workplace.
What to do:
• Compare the different models of assessment and their implementation in healthcare
• Discuss the application of theoretical perspectives to the care planning process in a healthcare setting.
• Review how the legislative and regulatory framework that supports equality and diversity is reflected in the assessment and care planning process
• Evaluate the influence of different theoretical perspectives on models of planning and assessment used in the workplace.
Part 2 (L02)
LO2 Implement care plans in the workplace to meet desired outcomes for individuals
LO3 Review the challenges and benefits of planning person-centred care in the workplace
LO4 Reflect upon the impact of the planning of care on practitioners, individuals, family and carers in relation to own practice
Scenario:
Whilst on work placement you will need to be involved in contributing to assessments and development of care plans. You will be assessed in your contribution to planning care and implementation of care plans in the setting, and complete a self-review of your practice.
Activity 2:
There are 3 parts to this activity where you would be required to create two real life case studies where you would create a care plan that would be implemented to meet the desired outcome of your service users or individual. After doing these two records of observations of your practice in promoting individual`s holistic care where you would review the challenges and benefits of planning a person - centred care in the workplace. Finally, a reflective account addressing the impact of the planning of care on practitioners, individuals, carers in relation to own practice.
1. Two real-life case studies that evidence your contribution to the care planning process and use of care plans in your workplace setting. You need to show evidence of a care plan you have used or been involved. (please anonymise all personal details) Your submission can include any other work products that evidence your contribution.
2. Two records of observation of your practice in promoting an individual`s holistic wellbeing through effective communication when implementing plans of care in your workplace (Witness testimony)
3. A critically reflective account that reviews of the care planning process and your provision of person-centred care, including the challenges faced in developing care plans that meet the needs of individuals and their required outcomes. Your review should be summative and suitably referenced. It should relate to personal development of ideas and experiences but should be underpinned by relevant and contemporary research.
A. Develop two case studies of individuals in your care (please anonymise all personal details) to provide a critical review of the challenges of developing care plans to meet the needs of the individual. Provide evidence of the care plan and answer the following questions. - See Appendix 1 for sample
• Discuss responsibilities and duties of own role in promoting person centred care planning in reference to the individuals in the two case studies created
• Demonstrate your own contribution to the care planning process and the use of care plan in setting (evidenced from care plan or witness testimony)
• Review the application of the risk assessment process in promoting person-centred planning in own workplace
• Critically review the challenges of developing care plans that meet the needs of individuals and their required outcome.
• Discuss the barriers to implementing care planning in the setting
• Review the benefits of the use of care plans in ensuring the needs of the individual are prioritised.
B. Two records of observation of your practice in promoting an individual`s holistic wellbeing through effective communication when implementing plans of care in your workplace (Witness testimony)- See Appendix 2
• Demonstrate your own contribution to the care planning process and the use of care plan in setting (evidenced from care plan or witness testimony)
• Demonstrate the promotion of an individual`s holistic wellbeing through effective communication when implementing a plan of care in own workplace. (witness testimony)
• Review aspects of the care planning process in terms of its impact on the individual, family and/or carers, including a discussion of how the individual and their family have contributed to the evaluation of the care process
• Explain the barriers to implementing care planning in the setting and discuss the strategies used to overcome barriers to effective planning
• Conclude with an overall review of the benefits of the use of care plans in ensuring the needs of the individual are prioritised.
C. Reflective account
• Provide a reflective account that Review aspects of the care planning process in terms of its impact on the individual, family and/or carers, including a discussion of how the individual and their family have contributed to the evaluation of the care process
• Critically discuss the importance of team work and leadership in developing supportive approaches for the individual and family in the planning of care
• Critically reflect upon the planning process in relation to own practice in the healthcare setting.
