You will need to address each requirement presented below in your report.

Assignment Requirements:

1. Identification of Programs or Projects:

• Identify any existing programs or projects designed to improve or enhance the healthcare needs of the designated area or population.
• Explain how these programs or projects aim to address these needs effectively.

2. Description of the Area/Population:

• Provide a detailed description of the designated area or population, including demographic information, prevalent health conditions, and existing service deficiencies.

3. Assessment of Significant Needs:

• Assess and prioritize the most significant healthcare needs within the selected area or population.
• Explain why these needs are considered the most significant, drawing upon relevant data and research.

4. Visual Representation (Self-Created):

• Minimum of 1 self-created visualization.
• Include a visual representation of your choice (e.g., graph, infographic, chart) to supplement the report`s findings.
• Ensure that the visual aids in better understanding the healthcare situation in the chosen area or population.

5. Data Sources:

• Use reliable data sources to support your analysis. Explore government health databases, local health department reports, and other reputable sources for information about the designated area or population.

6. Program Success Metrics:

• Identify and discuss specific metrics or indicators used to measure the success or effectiveness of the programs or projects in question.

Your report should be between ten (10) to twelve (12) pages in length, excluding the title page, reference pages, and any included illustrations.

 Follow APA 7th Edition Formatting guidelines. Ensure that your report is in Word document format, and include both a title page and reference page(s).

Provide a minimum of five (5) scholarly and/or credible references. These references should primarily be from the last five years, unless specific seminal works or course content references are used. Support the information throughout the report with evidence.
