INTRODUCTION: Mr Adam Waters is a 67-year-old man who was admitted to your ward (Surgical ward) two (2) days ago.

SITUATION: Adam has just been transferred back to your ward (Surgical A) by the Recovery Nurse, post transurethral resection of the prostate (TURP) in the operating theatre.

BACKGROUND: Adam’s father died at the age of 70 years from unspecified cancer (CA). Adam has been experiencing haematuria, dysuria and poor urine flow worsening for several months. During a recent follow-up of the results of the previous biopsy, PSA, MSU, and other blood tests at the local medical officer, Adam was diagnosed with prostate CA. He was subsequently referred to the urologist. After discussing treatment options, Adam elected to have Transurethral resection of the prostate (TURP). You have taken over the care of Adam in the surgical ward at BMDH during your clinical placement (as a second-year student RN), and the following is your assessment.

Question

1. Applying your knowledge of the pain pathway, describe the mechanisms resulting in Adam`s perception of reduced pain after being administered with Morphine 5 mg intravenously stat, following his TURP and discuss three (3) nursing responsibilities (giving rationales) of priority, within 30 minutes after administering morphine.

2. Identify and discuss two (2) non-pharmacological nursing interventions (giving rationales) when managing Adam`s post-TURP (within the 24hrs) pain taking into consideration his CBI. 

3. Identify and discuss three (3) possible complications for Adam, post-TURP and the associated nursing interventions of priority 72 hours after discontinuation of CBI.
